Inventory of the State Functional Assessment Tools for Long-Term Services and Supports

Functional assessment tools are instruments used by state Medicaid programs to assess applicants’ health conditions and functional needs when determining eligibility for long-term services and supports and create specific care plans for eligible individuals. This state policy compendium presents a 2015 inventory of assessment tools used in 50 states and the District of Columbia. It is organized into four sections: an introduction and methodology, summary data for each tool, definitions of terms used in the inventory, and links to documents used to compile the inventory.

Learn more about functional assessment tools and the themes that emerged from this inventory by reading the chapter from MACPAC’s June 2016 report, Functional Assessments for Long-Term Services and Supports.
