Functional assessment tools are instruments used by state Medicaid programs to assess applicants’ health conditions and functional needs when determining eligibility for long-term services and supports and create specific care plans for eligible individuals. This state policy compendium presents a 2015 inventory of assessment tools used in 50 states and the District of Columbia. It is organized into four sections: an introduction and methodology, summary data for each tool, definitions of terms used in the inventory, and links to documents used to compile the inventory.
